Do you wear contact lenses? If so, you’ll have to stop wearing them for about a month before getting a LASIK consultation. Contacts change the shape of your cornea, which changes your results when determining your LASIK candidacy. For best results, go with regular glasses for a little while.

Willingness to Sign a Consent Form
